Services Provided
Below is a list of services that are provided at A Real Difference. Our goals are to provide services to you so you can have the best care possible.
Day Habilitation
Our small and individualized Day Program includes Specialized Habilitation and Supported Community Connections. We engage individuals in a variety of functional activities with an emphasis on skill development and building relationships. A Real Difference offers this to DD and SLS funded individuals.
Mentorship
Assisting individuals with self-advocacy through instruction, modeling, advising or tutoring. A Real Difference offers this service to SLS funded individuals as a 1 on 1 staffing ratio.
Personal Care
Assisting individuals with daily living tasks like bathing, eating, dressing, grocery shopping, medical appointments and general life maintenance. A Real Difference offers this service to CES, EBD and SLS funded individuals.
Transportation
Service provided to enable waiver participants to gain access to waiver and other community services, activities and resources, as specified by the Service Plan.
Community Connector
The community becomes a learning environment where individuals have access to and participate in activities, functions and events. A Real Difference offers this service to comprehensive and SLS funded individuals in our Day Program, as a 1 on 1 staffing ratio or by Family Caregivers.
Homemaker Services (Basic & Enhanced)
Perform for or assist individuals with performing basic household tasks (i.e. cleaning and laundry) in the individual's home. A Real Difference offers these services to CES and SLS funded individuals as a 1 on 1 staffing ratio.
Supported Employment
Providing individuals with ongoing job related services and supports to help obtain and then maintain paid work in a community work setting. A Real Difference offers this service to DD and SLS funded individuals as a 1 on 1 staffing ratio.
Respite Care
Povide supervision and support to individuals in the absence of, or as relief to, the primary care giver. This service can be provided in or outside of the individual's place of residence. A Real Difference offers this service to CES, SLS and EBD funded individuals as well as Host Home Providers as a 1 on 1 staffing ratio
Individual Services and Supports (IRSS)
Different living arrangements assessed to meet each individual's needs. This may include living in a home owned or leased by a residential agency, the individuals own home or a Host Home. A Real Difference offers this service to DD funded individuals. We offer Host Home settings and will consider other situations on individual basis.
